Thank you for choosing Eye Columbus as your home for glasses, sunglasses, contacts, and eyecare (collectively called "Services").

We are very proud of the Services we offer our patients. The practices of Eye Columbus have been fortunate to provide these Services to the greater Columbus, Ohio community for generations. By using our Services, you are agreeing to the terms below. Please read them carefully.


A patient or customer of Eye Columbus is anyone that uses our Services. These may be used and interchanged throughout the following Terms of Service.

Patient Provider Agreement
Services provided by Eye Columbus are meant to enhance the quality of life for our patients, customers, and our community. As a patient at Eye Columbus, you acknowledge that you understand that the Services provided by the Doctors, Opticians, and Staff ("Eyecare Professionals") of Eye Columbus will be provided with the best interest of your ocular and visual health and performance in mind. Your Eyecare Professionals may prescribe a treatment plan to ensure the best potential of ocular and visual health and performance for you. This treatment plan may be in the form of prescription eyewear, medication, advanced testing, education, or other methods. It is the responsibility of the patient to follow through with this treatment plan as directed. If at any point you do not agree with a treatment plan, you should discuss it with your Eyecare Professional immediately.

Eye Columbus is a locally-owned, independent healthcare business. We rely on patients scheduling appointments and keeping those appointments in order to operate effectively. Scheduled appointments are times that we have reserved exclusively for you, our patient. We require a 48 hour notice via phone or email in the event you must reschedule or cancel an appointment. We reserve the right to apply a $25 administrative fee to any patient's account that does not keep the scheduled appointment and provide 48 hour notice to us. 

We offer many different ways to schedule an appointment at Eye Columbus including, but not limited to, phone, email, in-person, and online booking. By scheduling an appointment at Eye Columbus you are giving us permission to contact you via phone, email, text, or other methods as described in our Privacy Policy below. If at any time we need to change your appointment day or time we will contact you immediately and work to find a day or time that will work best for you.

Routinely, we schedule appointments months in advance ("Pre-Appointments"). These Pre-Appointments are designed to guarantee you a saved appointment time in the future. You will be contacted prior to this time to confirm that the saved appointment time still works for your schedule and provide you an option to change this appointment if necessary. 

Eye Columbus reserves the right to deny Services to anyone. No Eye Columbus employee will deny Services to a patient or customer without cause and without notification.

We do not require our patients to have vision or medical insurance to be seen at Eye Columbus. We believe that everyone has the right to have access to high quality eyecare and eyewear.  Eye Columbus is an in-network and out-of-network provider for many major insurance corporations. If a patient would like to use any insurance benefits towards the services or products provided, it is the responsibility of the patient to verify eligibility and provide us with all necessary information regarding your insurance pre-paid benefits. Eye Columbus will be happy to forward applicable charges to in-network insurance corporations on your behalf at no additional charge.

As your medical eyecare provider, Eye Columbus' best interest is in you, our patient, not your insurance corporation. We will work on your behalf to ensure your insurance corporation cooperates in a timely and ethical manner in accordance to our provider agreement contract. All copays, deductibles, and non-applicable charges are the responsibility of the patient and are due at time of services. Any charges denied by a patient's insurance corporation will be forwarded to the patient per the provider agreement contract. It is the responsibility of the patient to provide all valid insurance information to ensure proper billing and payment. While Eye Columbus is happy to assist and consult our patients on the matter of insurance corporation pre-paid benefits, it is the responsibility of the patient to review their specific vision or medical plan. If a patient wishes to question or dispute an insurance explanation of payment they may do so by contacting their insurance corporation's customer service directly. A $60 deposit at time of visit will be required to secure deductible amount when medical eye services are charged. This amount will only be applied if charges are to be forwarded to patient as instructed by your major medical insurance carrier. Any monies owed back to the patient will be refunded as outlined in the Refunds section below.

Patient Accounts and Financial Responsibility
Eye Columbus provides Services to promote preventative eyecare and to offer the best in ocular and visual health for you, our patient. Treatment may come in the form of prescribed products such as eyewear, contact lenses, medications or education. Excluding insurance billings as described above, payment for all Services are due in full at time of service and order. Fees for Services are subject to change.

Any patient accounts with a balance greater than 60 days old will be subject to a monthly $25 late fee administrative fee. Any accounts greater than 90 days old may be subject to collections upon notice. 

Eyewear Orders and Return Policy
Eyewear orders include all eyewear frames, lenses, and contact lenses. Eye Columbus offers premium quality eyewear and partners with third-party vendors in the manufacturing of the frames, lenses, and contact lenses. Eyewear purchased is custom made and ordered exclusively for you. The following return policy pertains to all eyewear and accessories purchased at Eye Columbus stores or online.
Contact Lenses: All unopened contact lens boxes my be returned for a full refund within 30 days of purchase assuming the boxes are as they were dispensed to the patient, unopened, and with the UPC codes intact.
Eyeglasses: All custom eyeglass orders, including sunglasses, may be returned for a partial refund within 30 days of purchase. Customers will be responsible for all incurred optical laboratory costs and professional services associated with custom eyeglass orders.
Accessories: All accessory orders such as eye drops, cleaning solution, cleaning cloths, vitamins, and other miscellaneous items are non-refundable.
Professional Services: All professional services provided by Eye Columbus are non-refundable. 

Any monies owed back to a patient will be refunded by check within ninety days. 

Privacy Policy

This privacy policy discloses the privacy practices for Eye Columbus and applies to information collected by Eye Columbus either in-person, online, or via telephone, text, email, and other communication methods. It also includes information collected by this website. It will notify you of the following:

  1. What personally identifiable information is collected from you through the website, how it is used and with whom it may be shared.
  2. What choices are available to you regarding the use of your data.
  3. The security procedures in place to protect the misuse of your information.
  4. How you can correct any inaccuracies in the information. 

Information Collection, Use, and Sharing
We are the sole owners of the information collected on this site and in Eye Columbus stores. We only have access to collect information you voluntarily give us. We will not sell or rent this information to anyone. We will use your information to respond to you regarding the reason you contacted us or to provide Services you requested from us. Some information will be shared to trusted vendors and insurance corporations in order to fulfill the Services you request from Eye Columbus. No information will be shared that is not required to provide these Services.

By requesting, receiving, or inquiring on Services at Eye Columbus you are giving us permission to contact you via phone, email, text, or other methods. This contact may be in the form of communications, updates, order status, confirmations, changes to these Terms, or to tell you about specials, new Services. You may ask us to stop contacting you at any time.

Your Access to and Control Over Information
You may opt out of any future Services or contact from us at any time. You also have the right, at any time, to request the following information from Eye Columbus. These request should come in the form of email or phone:

  • See what data we have about you, if any.
  • Change or correct any data we have about you.
  • Have us delete any data we have about you. Please note we are not allowed to delete any medical information pursuant to state laws.
  • Express any concern you have about our use of your data.


We take precautions to protect your information. When you submit sensitive information via the website, your information is protected both online and offline. Wherever we collect sensitive information (such as credit card data), that information is encrypted and transmitted in a secure way. If at any time you are worried about the security of your information we ask that you do no send it via electronic method and contact us. We will make arrangements that you can provide any necessary information in-person at one of Eye Columbus' store locations.

While we use encryption to protect sensitive information transmitted online, we also protect your information offline. Only employees who need the information to perform a specific job are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are kept in a secure environment.


Our Terms of Service may change from time to time and all updates will be posted on this page.
